We are always excited to welcome a new member to our “patient family” at Orthodontist of Cerritos. When you visit our modern and clean dental offices, we will start with a comprehensive oral exam and X-rays to see what’s happening under the surface. Once we have completed our assessment, we will go over any recommendations with you and your treatment plan.

Review the case and determine a course of treatment.

OUR ACCEPTED INSURANCE PLANS

We accept most Preferred Provider Organization (PPO) dental insurance plans. We are sorry, but we do not accept Medi-Cal, DentiCal, or HMO at this time. However, we do offer affordable financing options.

NEW PATIENT FORMS

Save time at our Cerritos orthodontist office by filling out your patient forms before you arrive. To make this convenient, we have made them available for you online.

These patient forms should be completed for every member of your family that is coming in for an orthodontic consultation. If filling out a form for your child, please try to remember to let us know about any special circumstances, including other diagnoses or conditions. We also need to know if you or your child are on any medications. Our patient forms are treated confidentially to meet all federal HIPAA privacy requirements.
